HEALTHY HABITS

People with long-term lifestyle-related health conditions feel more empowered when they play a greater role in managing their care.

 

Healthy habits is a diary and card set for those who have type 2 diabetes. Writing about their feelings can help people with these conditions overcome emotional upsets and feel happier.

The first section allows users to monitor the five factors that influence type 2 diabetes: activity, diet, sleep, stress and blood glucose levels. Users can then work out what areas require more attention, and create goals and targets accordingly.

The next stage then allows them to create their goals to develop new habits that will enable them to manage their diabetes and feel more empowered. The diary is broken down into days and weeks, so users can take small steps to begin with.

 

Along with the diary comes a set of cards: goal cards, habit cards and ‘SMART’ cards. The goal cards explain what goals are, the different types of goals and their benefits. The habit cards give users tips on how to start creating habits. These cards can also be removed from the booklet for users to keep with them as a reminder. The ‘SMART’ cards stand for specific, measurable, achievable, realistic and timed goals. These are the key aspects of setting good goals, and include helpful examples.
